More complete informations about this job opportunity kindly read the description below. JOB SUMMARY: Consults and coordinates care of patient with physician, program coordinator and agencies. Assesses treatment planning and progress. Prepares trea! tment plan and goals for each individual resident/patient. Provides crisis, assessments and intervention. Maintains pertinent patient/resident data in medical record in accordance with state and guidelines. Offers in-service education programs and functions as a resource to other staff and the community as needed. Participates in development and selection of patient education materials and services. Provides individual, family, and group therapy for patients/residents and assists other staff with group materials. Performs other duties as assigned This person’s work will be appropriately monitored/supervised and supervision is documented in writing, per state regulations.
ESSENTIAL DUTIES AND RESPONSIBILITIES Investigates case situations and presents information to Psychiatrist/Addictionologist and other members of health team, on patient's family and social background pertinent to diagnosis and treatment. Helps patients to respond constructively to treatment and assis! t in adjustment leading to and following discharge. Explains B! ehavioral Health treatment to patient's family and helps to reduce fear and other attitudes obstructing acceptance of psychiatric care and continuation of treatment. Serves as link between patient, psychiatric/addiction service agency, and community. Works directly in treatment relationship with patients, individually or in groups, in consultation with Psychiatrist and program Team leader. Refers patient or patient's family to other community resources.
EDUCATION, TRAINING, & EXPERIENCE Master's degree in Social Work, Guidance and Counseling, Marriage and Family or other state recognized Behavioral Health degree.
CERTIFICATION/LICENSURE Applicant has accomplished a Masters degree in a mental health/behavioral health field with documented clinical course work and field experience. Person is working actively on a plan toward obtaining clinical hours and state licensure as a LCSW, LMFT or LPC.
OTHER CERTIFICATION/LICENSURE TACT Certified within 30 days after hire (! or per programs guidelines) CPR Certified-within 90 days after hire (or per program’s guidelines) First Aid Certified-within 90 days after hire (or per program’s guidelines) Maintain a valid driver license-preferred ( or per programs guidelines)
OTHER SKILLS AND ABILITIES Applicant must possess a broad background of training and experience with various patient populations and modes of service delivery. TACT certified and able to assist with TACT if needed.
SPECIAL PHYSICAL DEMANDS While performing the duties of this Job, the employee is regularly required to sit; use hands to finger, handle, or feel; reach with hands and arms and talk or hear. The employee is occasionally required to stand; walk and stoop, kneel, crouch, or crawl. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision and ability to adjust focus. Assist with TACT in emergency situations .
Language Skills: Ab! ility to read, analyzes, and interprets general business periodicals, p! rofessional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.Mathematical Skills
Ability to add, subtracts, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.Reasoning Ability
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.Computer Skills
Requires a high level of independent judgment. The position has the potential for substantial latitude and independent action.
