The Bachelor Level Clinician-Substance Abuse Counselor (B/SAC) completes assessments, case management and discharge! planning of clients admitted to the Behavioral Health Unit for mental health or alcohol/drug services. This position assists the Internal Medicine physicians as needed in their evaluation of AODA patient and/or the Psychiatrist or Psychiatric Nurse Practitioner in evaluation of mental health patients. This includes but is not limited to completion of appropriate psychosocial evaluations, assist in leading department groups, and case management/discharge planning, with appropriate referrals based on client needs. The B/SAC assures that an individualized Treatment Plan is established for each patient and that a Treatment Plan Review is done weekly for patients that remain hospitalized for greater than 72 hours. Daily work consists of attending patient rounds and meeting clients with the psychiatrist to complete a psycho-social evaluation for adults and children ages 13 and older. The B/SAC is a strategic member of the Treatment Team and makes decisions and recommendations in! the area of social services required, placement in other soci! al structures, counseling of family members related to the needs of the patient and family with recommendation for follow-up services. Work is performed in accordance with established departmental policies with application of professional knowledge and skills in completion of case assignments, working with a relative degree of professional independence under the direction of the Master Level Social Worker, Psychiatrist & Internal Medicine Physician. Performance of Substance Abuse Counselor duties are under the supervision of the AODA Service Line Administrator.

Clinicians hired in this position will have responsibilities in the Crisis Center including conducting Mobile Crisis assessments, answering crisis calls and triaging walk-in clients for admission to Behavioral Health Hospital, Crisis Stabilization or Outpatient services.

The B/SAC assures that care is provided in a professional manner that maintains the dignity, respect, and rights for all patients.! The B/SAC collaborates with the treatment team, assisting to develop an individual treatment plan in conjunction with the Psychiatrist and other interdisciplinary treatment members, the patient, and family. The B/SAC facilitates group therapy sessions for patients on the unit.

This is a nearly full-time position (36 hours per week) in our Emergency Services, primarily day shift however can be variable in hours; every other weekend required.

QUALIFICATIONS:
Required Bachelor’s Degree in Social Work, Psychology, Sociology, or a related field.

Required Substance Abuse Counselor (SAC) or Substance Abuse Counselor In Training (SAC-IT) Certification.

Minimum one year experience preferably in an acute psychiatric setting.

Valid Wisconsin Driver’s License.

No prior felony convictions for crimes relating to harm to another person, theft, misuse or misappropriation of a legal or illegal drug.

Must pass cri! minal background check.

Must be physically able to perform wor! k duties..

Must be able to follow written and oral direction in English.

Must provide proof of employment eligibility.

DESCRIPTION OF JOB:
Responsible for timely completion of psychosocial assessments, Treatment Plan and Treatment Plan Reviews, and Discharge Summaries.

Complete the Hospital Substance Abuse evaluation form within 24 hours of client admission.

Completes an individualized Treatment Plan for each patient after72 hours, including short and long term goals after assessment of the patient.

Responsible for appropriate Case Management/Discharge Planning of assigned patients based on patient need.

Documents/dictates daily pertinent information about patient behaviors, needs, moods, and progress and in a timely manner.

Coordinates programming or follow-up as applicable with the outpatient services and other agencies as required for continuum of patient care.

Completes Uniform P! lacement Criteria for determining AODA level of care

Collaborates and communicates effectively with the interdisciplinary team. notifying the Physician and/or Charge RN immediately of significant changes in patients’ conditions and/or critical incidents.

Makes appropriate and timely referrals to other agencies based on patient need and services required.

Assists in the completion of patient aftercare forms.

Facilitate patient transfers to Nursing Homes and/or other assisted living facilities (Group Homes, CBRFs) including appropriate discharge paperwork and after care planning, including scheduling appointments.

Completes crisis assessments and crisis plans for Chp-34 Emergency Mental Health Service Program

Makes treatment recommendations to county corporation counsel and Human Service Departments.

Knowledge of the principles and practice of professional social work and the ability to apply this knowledge in t! he assessment and evaluation of individual clients in determining the ap! propriate plan of care.

Knowledge of basic sociology and psychology as it applies to mental illness and crisis services.

Knowledge of area resources, programs and services for client referrals.

Knowledge of dealing with client behavioral problems.

Ability to apply knowledge to individual clients and to relate professional training to real-life situations.

Ability to establish rapport with clients, their families and associates so as to be able to begin to help them meet their social needs.

Ability to write and prepare concise case histories, summaries, updates and to write clear and concise correspondence and to keep accurate records.

Maintains attendance at an acceptable level as outlined in the facility Attendance Policy.

Maintains confidentiality and protects sensitive patient data at all times.

Demonstrates bachelor level social work competency in professional work and engages in opportunit! ies for professional growth and continuing education.

Assures that patient care provided in the department is completed in a respectful manner that upholds the dignity and rights for adolescent and adult patients.

Performs other duties as assigned.

COMPENSATION:
Competitive hourly wage based on Market Average and experience.
