More details about this career opportunity kindly read the description below. JOB SUMMARY: Consults and coordinates care of patient with physician, program coordinator and agencies. Assesses treatment plannin! g and progress. Prepares treatment plan and goals for each individual patient/resident. Provides crisis, assessments and intervention. Maintains pertinent patient data in medical record in accordance with state and guidelines.
Offers in-service education programs and functions as a resource to other staff and the Community as needed. Participates in development and selection of patient education materials and services. Provides individual, family, and group therapy for residents and assists other staff with group materials. Performs other duties as assigned.
ESSENTIAL DUTIES AND RESPONSIBILITIES: Investigates case situations and presents information to Psychiatrist and other members of health team, on patient's family and social background pertinent to diagnosis and treatment. Helps patients to respond constructively to treatment and assist in adjustment leading to and following discharge. Interprets psychiatric treatment to patient's family and helps to! reduce fear and other attitudes obstructing acceptance of psy! chiatric care and continuation of treatment. Serves as link between patient, psychiatric agency, and community. Works directly in treatment relationship with patients, individually or in groups, in consultation with Psychiatrist and program coordinator. Refers patient or patient's family to other community resources.

EDUCATION, TRAINING, & EXPERIENCE: M aster’s degree and appropriate level of field experience and coursework. Child and/or AODA expertise preferred.
CERTIFICATION/LICENSURE: Licensed to independently provide psychotherapy in Wisconsin (includes LCSW, LPC, LMFT, Licensed Psychologist) and certified as a Medical Assistance provider.

CPR Certified-within 90 days afte! r hire
First Aid Certified-within 90 days after hire
Fire Safety Certification-within 90 days after hire
Maintain a valid driver license-preferred
OTHER SKILLS AND ABILITIES
Applicant must possess a broad background of training and experience with various patient populations and modes of service delivery.
WORK ENVIRONMENT
The noise level in the work environment at times is high and chaotic.
Occupational exposure is category C that means no employees in the specified job classifications have occupational exposure. Must be TACT certified and able to assist when necessary.

Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
Mathematical Skills
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpre! t bar graphs.
Reasoning Ability
Ability to solve practical! problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Computer Skills
Requires a high level of independent judgment. The position has substantial latitude for independent action.
