Under the direction and supervision of the Psychiatric Services Director and the Clinical Director, the Outpatient Psychiatric Clinic Nurse is responsible for ! providing nursing support to outpatient psychiatric services including; psychiatric services provided to clients with mental health, AODA & developmental disabilities. The nurse is responsible for a variety of duties utilizing nursing skills when triaging and assessing clients. This position requires the nurse to use solid nursing judgment within the scope of the nursing discipline, adhering to clinic and organizational policies, and adhering to patient confidentiality regulations.

This is a part-time position, Monday-Friday, Day/PM varied shift in our Outpatient Psychiatry Services. 32 - 40 hours per week with no weekends.

QUALIFICATIONS:
Graduation from an accredited school of nursing required.

Current and valid registered nurse license by the State of Wisconsin Board of Nursing required.

One year required experience with psychiatric nursing, psych medications, crisis intervention, a knowledge base of mental health, alcohol/d! rug, developmental disabilities and work with children/adolesc! ents. Additionally, having knowledge of WI State Statute Chapter-51, HFS Chapter-34 is of considerable advantage.

Current CPR Certification.

Must pass criminal background check.

Must be physically able to perform work duties.

Must be able to follow written and oral direction in English.

Must provide proof of employment eligibility.

DESCRIPTION OF JOB:
Provide nursing care in a respectful manner that upholds the dignity and rights of clients.

Maintain confidentiality of client and medical records.

Provide nursing care under the direction and orders of the Psychiatrist, Physician, Nurse Practitioner, or Physician’s Assistant.

Serve as a liaison between the client and provider to address client concerns and needs.

Triage and document clients for emergent and non-emergent appointments using a detailed nursing assessment.

Provide clients with a detailed phone assessmen! t. Document the assessment.

Document all client contact and interventions in client’s medical record.

Notify the physician or client’s therapist of reported changes in a client’s condition.

Assess, coordinate and manage client care utilizing the nursing process, which includes assessment, planning, implementation, and evaluation.

Complete physician orders as directed, consult with the physician promptly if there are any questions or identified discrepancies.

Administer and document medications after obtaining a signed order from the physician and client’s consent to administer medication following the 5 rights of medication administration (Right patient, route, dose, time, and medication.

Assist client in completing medical histories and medication lists.

Educate client and/or guardian on medication, medication side effects, and medication financial assistance programs.

Obtain medication info! rmed consents: verbal and written.

Maintain, stock, and tr! ack pharmaceutical drug samples in an organized manner in the locked medication room and document distribution of samples to client as ordered by provider.

Assist client(s) in obtaining necessary information to enroll client in patient assistant programs.

Assist provider in completing prior authorizations related to medication and service. Return completed prior authorization to pharmacy or insurance company.

Assist client and provider in completing and mailing mail order prescriptions.

Obtain patient vitals as indicated by provider.

Complete PASSAR screens as needed.

Complete AIMs screenings as needed.

Assist provider in client requested form completion to include: FMLA, DOT, SSI, work excuse, school excuse, etc.

Document nursing care, observations, behavior, and interventions in accordance with standards of nursing care and department/organizational policies and procedures.

Ensure client’s ! required lab work is completed as ordered. Review lab values and contact providers and client as needed.

Obtain group home prescriptions and samples from provider.

Serve as a liaison between provider and jail staff in order for mutual clients to receive continuity of psychiatric care and medications.

Coordinate and prepare for Telehealth services with provider, client, and staff. Prepare prescriptions and labs, and coordinate follow-up care.

Prepare paper prescriptions per client for providers as needed.

Initiate provider ordered referrals for client psychiatric care. Follow-up on referral as requested by provider.

Assist in coordinating outpatient psychiatric services transfer of care when necessary.

Collaborate with NCHC Hospital as needed to meet medication related needs upon patient’s discharge.

Coordinate distribution of psychological testing materials.

Score (not interpret) psychologica! l testing as requested by provider.

Access and utilize electro! nic medical records.

Attend required meetings.

Collaborate professionally with nurses from other departments, assisting and providing coverage as needed and requested.

Assist in writing triage guidelines.

Identify opportunities for improvement and make recommendations to improve services and/or develop processes/protocols related to improving the client experience and client/staff/environment safety.

Maintain a professional work environment.

Perform other job duties as assigned.

Knowledge of client care practices and standards acquired through graduation from an accredited registered nursing program and licensed by the State of Wisconsin to practice professional nursing care.

Knowledge of psychiatric diagnosis, medications, and behaviors to provide care to clients in outpatient psychiatric services department.

Use effective listening and communication skills (verbal and written).

Abili! ty to prioritize duties and complete work within designated timeframes.

Ability to remain calm and effective in emergency situations

Provide a safe environment and promotes quality patient care through adherence to established standards of care and facility policies and procedures.

Engage in opportunities for professional growth.

Contribute to the productivity and professional environment of the department.

Adhere to OSHA guidelines.

Adhere to the facility Attendance Policy in addition to all facility policies, procedures and regulations in administering psychiatric care to clients.

Complete incident report and report incident to direct supervisor.

Demonstrate effectiveness in ongoing patient support by adhering to safety, fire and disaster, infection control and hazard communication policies and procedures with completion of annual competencies.

Complete conditions of employment on time.
