DUTIES:
Make practice decisions autonomously and/or in consultation with physician(s) and health care professional(s), in a collegial relationship for mutual consultation and referral. The APN is responsible and accountable for own actions.
Collaborate with the patient/family to establish physical, mental, cultural and environmental factors that facilitate or impede realistic goals and activities.
Conduct and document findings on initial, periodic, episodic, and/or psychiatric examinations on patients seen in the clinical setting.
Determine potential needs for therapeutic changes, diagnostic workups or subspecialty consultation and so order.
Order and/or interpret appropriate laboratory tests.
Provide follow-up for evaluation of outcomes of laboratory tests. Detect and evaluate any significant change in patientsÂ' psychiatric condition, treating as indicated.
Review medications for clinical need, simplification and possible drug-drug interactions and adjust as indicated.
Prescribe, administer and review/evaluate patientÂ's response to pharmacological and non-pharmacological treatment.
Evaluate outpatients and manage acute and chronic mental illnesses according to the usual scope of care to include attention to maintenance of stable conditions, preventive health care and screening. Collaborate with collaborating physician or other allied health care professionals as needed.
Formulate diagnostic plans and therapeutic regimen, including possible hospitalizations. Collaborate with collaborating physician or other allied health care professionals as needed.
Manage and consistently reassess home care patients with stable acute/chronic health care problems, including common diseases and human responses to illness states.
Plan, provide and evaluate care that promotes wellness and prevents disease/injury and disability.
Assist patient to maintain their current state of health when psychiatric disability already exists.
Assist patients to regain or improve their abilities to cope with states of health and illness.
Serve as a mentor, consultant and coordinator for nurses and other health care professionals.
Provide education and counseling of the patient and the patientÂ's family in preventive care, psychiatric conditions and the use of prescribed treatments and drugs. Maintain phone contact with patients, families and home care agencies as indicated.
Maintain updated Problem List and current health status for each patient visit as evidenced by interim progress notes and summaries in patientÂ's medical record.
Coordinate health care serviced; participate on clinic committees and professional groups in the community (consultation).
Clean RME (Reusable Medical Equipment) per SOP (Standard Operating Procedures).

Physical standards - All applicants must meet the physical standards for the position. A physical examination prior to employment may be required.
Registration - Current, full, active and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the U.S. or in the District of Columbia . The RN must maintain a current, full active and unrestricted registration to continue employment with the VA
Educational requirements can be met as follows:
(1) Graduate of a school of professional nursing approved by the appropriate State agency, and accredited by one of the following accrediting bodies at the time the program was completed: a) The National League for Nursing Accrediting Commission (NLNAC); or b) The Commission on Collegiate Nursing Education (CCNE). OR
(2) The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. OR
(3) In cases of graduates of foreign schools of professional nursing, possession of current, full, active, and unrestricted registration, will meet the requirement of graduation from an approved school of professional nursing.
The following requirements are preferred for this position:
At a minimum, applicants should possess experience as an RN that is determined to be sustained and consistently at or above an acceptable level of quality. This may include experience as a Graduate Nurse Technician (GNT) provided that candidate was utilized as a professional nurse and passed the State licensing (board) examination on the first attempt, and experience as Nurse Technician Pending Graduation provided the candidate possessed an active, current registration to practice nursing in a State and was utilized as a professional nurse. Professional nursing experience should be documented on the Application for Employment - Nurse and Nurse Anesthetists (VA Form 10-2850a).

Salary determination will be based on a recommendation by the Professional Standards Board and approved by the Medical Center Director.
APRN QUALIFICATIONS:
1. Must be a Citizen of the United States.
2. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE).
3. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
4. Must be proficient in spoken and written English.
Additional requirements for Advanced Practice Nurses (APRN): An APRN must be licensed or otherwise recognized as an APRN in a State, possess a masterÂ's degree from a program accredited by the NLNAC or CCNE, and maintain full and current certification as an APRN from the American Nurses Association or another nationally recognized certifying body.
Applicant's education and length of nursing practice (experience) will be considered by an Advance Practice Nurse Professional Standards Board in determining the grade and salary of the applicant selected. Experience as a APRN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry . Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel. The criteria are as follows: Practice: practice, ethics and resource utilization; Professional Development: performance, education/career development; Collaboration; collaboration, collegiality; and Scientific Inquiry: quality of care, research.
The Advanced Practice Nurse III must demonstrate performance and leadership that is broad and complex enough to improve the care for a group of patients and document outcomes at the program or service level. This would include leadership in ethics, resource utilization, education/career development, performance, collaboration, collegiality, quality of care and research. Documentation of outcomes is required in consideration for appointment or promotion.
