Responsibilities:
The Behavioral Health RN Care Manager functions as a core member of a Collaborative Care team. The care manager is responsible for coordinating and supporting mental health care for patients whose conditions are managed by primary care providers. The care manager role educates patients about depression and its treatment and monitors depressive symptoms and patient’s response to medication and/or psychotherapy using a structured instrument (e.g., the PHQ-9). The care manager works closely with the primary care providers to coordinate care with psychotherapists and consulting psychiatrists. Other duties as assigned.
Summary:
A minimum of 3-5 years of clinical experience in a psychiatric setting is required. Experience with depression and depression treatment is preferred. Experience working with patients who have co-occurring mental health, substance abuse and physical health problems is preferred. Experience working with medically ill and/or older adults and clinical experience assessment and treatment planning for common mental health and/or substance use disorders is desired. Prior exposure to brief, structured counseling techniques. Bachelor's degree in Nursing is required. Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact). American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required upon hire.
