Provide a variety of therapeutic activities including the initial intake and assessment of patients, coordination of appro! priate referrals for patients and assess walk-in patients.
In addition, provide assistance with medication refills, and prior authorizations.
Provide an active presence in the multidisciplinary team.

ESSENTIAL DUTIES
1.
Evaluate patients and render care, within the scope of practice outlined by state statute, in the geriatric psychiatric clinic.
2.
Perform direct patient care activities, including assessment, evaluation, and treatment.
3.
Maintain detailed, confidential medical records in accordance with established protocols.
4.
Communicate with team members.
5.
Complete refill requests, prior authorizations, and communicate with pharmacies and third party payers.
6.
Respond to patient phone calls regarding medications, questions, and crisis situations.
7.
Assist with the triage of walk-in patients and patients in crisis.
8.
Provide crisis intervention as needed for current clinic patients and new referrals.
9.
Provide case management services t! o coordinate independent living needs, social service needs, and other mental health needs.
OTHER DUTIES
Perform other duties as assigned.
MINIMUM POSITION QUALIFICATIONS
Appropriate education and/or experience may be substituted on equivalent basis
Education:
Associate's Degree in Nursing.
Bachelor’s Degree in Nursing preferred
Experience:
No experience required
Certifications/Licenses:
Current Wisconsin Licensure as RN
KNOWLEDGE, SKILLS, & ABILITIES
Strong leadership, team-building and decision-making capabilities are essential, as well as the ability to set priorities, identifies and resolves problems, and delegate effectively and efficiently.
Excellent communication and organizational skills needed to manage workflow, patients, patient families, physicians, and others under stressful conditions.
The ability to assess and triage p! atients and to communicate clearly and logically both orally and in wri! ting needed.
Must demonstrate reliability and maintain confidentiality.
Demonstrated proficiency in the use of the electronic medical record (EMR).
